Allergies

Students who have life-threatening food allergies will need to provide a completed Allergy Action Plan to the school nurse.  The form requires a physician's signature, as well as a parent/guardian signature.  Please provide the child's epi-pen and/or allergy relief medication to the school nurse in a timely manner to ensure your child's safety at school.  Allergy Action Plans are effective for one school year.  New orders are required at the beginning of each school year.

The Food and Services department will make modifications to the regular school meals for a student with a disability that restricts his/her diet.  This requires a physician to complete and sign the Special Diet Prescription Form.  For more information, click on the link below:
